1: Raise initial failure report

Test Phase; Operation Phase, Fault Symptoms; Date of Occurrence; Log number: Detailed description of Failure effects

2: Complete "On Equipment" Data

  • For WRA and/ or SRA: Part number; Nomenclature; Serial Number; Elapse Time Meter reading (if available);
  • Summarize the maintainability process
  • Summarize the adequacy of maintainability provisions
  • Record any improvements or recommendations for the maintainer and possible maintainability characteristics.

2a: Complete "On Equipment Maintenance" Data

  • For system level h/w failures (other than WRA/ SRA) enter details of repair action
  • Summarize the maintainability process
  • Summarize the adequacy of maintainability provisions
  • Record any improvements or recommendations for the maintainer

3: Complete "Off Equipment Maintenance" Data

  •  For SRA: Part number; Nomenclature; Serial Number;
  • Summarize the maintainability process
  • Summarize the adequacy of maintainability provisions
  • Record any improvements or recommendations for the maintainer and possible maintainability characteristics

4: Annotate Close Out Action

Annotate the Close Out Action with the disposition stating the cause of the system/ test failure. (other than a hardware failure e.g. operator error, test procedure error etc.). Include date.

5: Annotate Close Out Action

Annotate the Close Out Action with the disposition stating that the "Failed Unit (WRA or SRA) has been return to Customer for repair". Include date.

6: Summarize Vendors Failure Report

Review the vendors failure report, include a summary of the failure cause into the original failure report. This will include the identification of failed component(s) with circuit designator numbers etc.

7: Close Out Action

Summarize in the Close Out Action, the disposition taken to resolve an unwarranted failure trend/ problem e.g. the corrective action.

8: Annotate Waiting

Annotate the Failure Report that the vendor will supply a detailed failure report and is required prior to closing out the Original Failure Report
